A letter verifying you are currently in good standing in the PharmD program, as generated by the office of the Associate Dean for Student Affairs.

Please note: To facilitate your application to the UC PharmD-MPH Program, the PharmD-MPH Admissions Committee will access and review the letter of standing and essay you are submitting.

Further, please be aware that in accordance with FERPA, de-identified applicant data required by the MPH and college's accreditation body will be shared, as required, for accreditation purposes only.
